Family Direct Support Professional Job Description Job Overview: This position is responsible for providing individualized support, skills training, positive direction, and assistance to individuals who experience a developmental disability in accordance with state and program policies and procedures and their own personal outcome and goals.  Outcome: To assist and coach an individual to achieve their goals and desired outcomes and to live successfully independently. Employment Requirements: * Minimum of 20 hours a week * Must be 18 years of age, or older * High school graduate or equivalent * Additional experience with people who experience developmental disabilities preferred * Must possess a valid driver’s license, access to a reliable vehicle, current auto insurance, and good driving record * Clear background checks as defined by state regulations and policies * Ability to do daily standing, walking, bending, and lifting (up to 20 lbs) * Must possess good verbal, reading, and written communication in English; basic math skills; and basic computer skills * Ability and willingness to successfully complete and apply training * Interest in assisting people supported to achieve personal outcomes and to provide person centered supports * Ability to work effectively and harmoniously with others; be of good moral character; have an interest in the welfare of the individuals; have a willingness/ability to communicate with and relate to individuals, parents, guardians, fellow-staff, representatives of community agencies, and company management * Flexibility with respect to hours of work and changing conditions * Additional certifications may be required by state regulations * Exhibits a talent for: * Inclusion: Ability to accept others where they are at, as they are, and actively include them for the purpose of helping them engage in life to its fullest * Relating: Enjoy getting to know others and working with them to achieve a common goal * Communication: Ability to consistently and reliably communicate with all members of their team in an effective manner * Responsibility: Exhibits the values of honesty and integrity by following through with their word  Responsibilities: * Provides supports to individuals by assisting in the development and implementation of all support plans, including assisting people in achieving their personal goals and desired outcomes as outlined in their service agreement and provide interaction and choices for activities that support their goals, and supporting people to be independent in activities of daily living * Assists with information gathering for Personal Outcome Measures, and other assessments * Assists people with maintaining a clean, safe, and orderly home by following safety and infection control procedures; educates people about maintaining homes * Assists people with getting to appointments, errands, and to various locations in the community by driving them to and from said locations * Maintain and expand skills and knowledge relevant to providing high quality support and services * Obtain and maintain all required certifications and trainings and apply the information to support people served * Maintain open communication and cooperative work effort with management, staff, customers, and other support staff to assure quality and continuity of support to people served * Supports the ACC Mission and Core Values while empowering each person to direct their own life choices * Completes proper and timely documentation including daily logs, incident reports, progress notes, etc. in a manner that meets or exceeds the requirements of ACC, brokerages, and/or state regulations while with the customer * Assists people with fiscal management while safeguarding their funds. This may include assistance in balancing checkbooks, paying bills, or budgeting * Ensures that people exercise their rights and that rights are not restricted without due process, ensuring individuals are treated with dignity and respect * Performs other duties as assigned
