The Charles Lea Center in Spartanburg, SC is currently hiring Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) for full-time and PRN positions. Your responsibilities will include: •    Providing services and supports focused on people achieving goals while being person centered. •    Provides individual care according to their individualized plan and individual needs as directed by the patient care and/or doctors’ orders. •    May administer medications in CLC’s ICF- IID home if the need arises following CLC procedures. Follow policy and procedure for correct administration and documentation. •    Communicates with the Program Manager/Lead Nurse of significant findings or changes in the patient’s physical or mental condition. •    Practices within the limits of the Nurse Practice Act and understands legal consequences of nursing action. •    Attend medical appointments as needed •    Complete assessments and medication audits as needed and assigned. •    Assess and make recommendations for individuals cared for in compliance with the Nursing Practice Act •    Participate in developing and implementation of individual’s program plans. Include any health-related training needs of people being supported and staff supporting them. •    Attend team meeting when medical issues arise to seek resolutions and recommendations on medical and/or health related concerns. •    Provide training on any new adaptive equipment and its uses to ensure staff are using the devices properly- ex: feeding devices, lifts, wheelchairs, communication devices, shower chairs, special shoes, spoons, plates, cups, clothing etc. •    Provide training & assistance to staff and people supported on an on-going basis and ensure appropriate documentation is completed •    Provide hands on support to people supported and become a part of the facility team (may help with meals, feeding, bathing, dressing, toileting, basic hygiene, recreational activities, and rehabilitative exercises) This provides an opportunity to develop staff. •    Complete med pass check offs on new staff when needed to ensure they are competent and compliant in CLC Med tech policies. Complete PRN med pass check offs and conduct training as issues arise. •    Provide on-call support to facilities and advise staff on appropriate treatment and care. May need to make an on-site visit as needed to determine if person needs emergency medical care or to complete follow-up. •    Provide training to staff and people supported including documentation. •    Responsible for giving injections (n/a to insulin), provided complicated wound care, trach care, peg tube feedings, and other care as needed for people supported. •    Take verbal orders from physicians until written orders are obtained and follow up to ensure written orders are obtained. Make needed corrections on MAR for any verbal orders received and assist the PM in making sure the medications/treatments are available within 24 hours. •    Work a flexible schedule to allow time to visit with people receiving services to determine if there are any unmet needs. What You’ll Need: * Must be at least 18 years old * Diploma or Associates degree from an accredited nursing program * Active license in good standing with the SC Board of Nursing * CPR and First Aid certification * Strong time management and communication skills * Ability to travel frequently within Spartanburg County * Valid driver’s license and Social Security number * Ability to type 70+ WPM and navigate basic computer systems * Must pass background checks, DMV screening, Medicaid exclusion, fingerprinting (if SC resident <1 year), and provide 3 professional references Work Environment Primary duties may be conducted in a remote setting with the flexibility and ability to work at a specific CLC location as/if needed. CLC’s interdepartmental Remote Work Policy will apply. This position involved distribution of medications in a variety of settings which may require frequent walking. This position may also come in contact with patients who have contagious illnesses. Physical Requirements: This role involves regular standing, walking, lifting (up to 50 lbs), and hands-on support.
