POSITION SUMMARY The Adult Psychiatric Rehabilitation Program (PRP) Counselor plays a vital role in supporting adults as they build the practical life skills needed for greater independence, emotional stability, and community functioning. This is a hands-on, community-centered position ideal for someone who loves helping others grow in real and meaningful ways. Each day, the PRP Counselor works directly with clients in their homes, communities, or via telehealth (when allowed), helping them practice daily living skills, improve social interactions, manage emotions, organize tasks, navigate community resources, and achieve the goals outlined in their Individual Rehabilitation Plan (IRP). This is not an office-based job PRP Counselors are active, mobile, and deeply engaged in clients’ day-to-day lives. A typical day includes: * Meeting clients in the community to practice real-life skills * Supporting emotional and behavioral stability through coaching * Helping clients manage appointments, transportation, or community activities * Teaching coping skills, communication strategies, and problem-solving * Documenting progress to meet Medicaid and program standards * Collaborating monthly with the client’s therapist to align skill-building with treatment goals ESSENTIAL DUTIES & RESPONSIBILITIES SKILL-BUILDING SERVICES * Deliver psychiatric rehabilitation interventions based on the IRP. * Provide services in the community, home, office, school, or telehealth (as approved). * Support skill-building in: * Emotional regulation * Hygiene & daily living skills * Social skill development * Communication & problem-solving * Wellness management * Recreation and leisure * Community living skills * Health, nutrition, Somatic management and counseling * Co-occurring disorders * Connecting with community resources * Social involvement, Community reintegration * Money management and budgeting skills * Managing health concerns * Household management * School or community functioning * Time management & organization * Medication monitoring and education * Coping skill building * Anger management skill building * Coordination of care with the members' treatment teams * Vocational and educational support * Technology and Digital literacy * Transitions and Life Changes COLLABORATION & CARE COORDINATION * Attend and document monthly collaboration with the OMHC therapist. * Communicate risk, behavioral concerns, and progress. * Participate in treatment planning, team meetings, and staff meetings. DOCUMENTATION & COMPLIANCE * Complete DLA 20 assessments, monitor and document progress * Complete session notes within required timelines. * Complete monthly summaries and Medicaid-required forms. * Complete collaboration documentation * Complete entitlement applications, resource, and referrals applications. * Maintain compliance with CARF, COMAR, Medicaid, and ARHC standards. RISK MANAGEMENT & SAFETY * Report all safety issues immediately. * Participate in crisis planning and safety interventions. * Follow mandatory reporting laws. GROUP SERVICES * Facilitate or co-facilitate PRP groups as assigned. * Document participation and progress accurately. ETHICS & CLIENT RIGHTS * Maintain confidentiality and trauma-informed practice. * Ensure accessibility, dignity, and cultural responsiveness. * Maintain professional boundaries at all times. KEY PERFORMANCE INDICATORS (KPIs) * Completion of the required visit frequency based on IRP goals * Documentation timeliness & accuracy * Report IRP goal progress * Collaborative compliance with the client's treatment team * Safety and incident reporting compliance * Client engagement trends TOOLS, EQUIPMENT & TECHNOLOGY * InSync EHR * Agency communication systems * Telehealth platforms * Secure mobile device (if applicable) PHYSICAL REQUIREMENTS * Ability to travel in the community * Ability to complete visits in homes/schools * Ability to sit, stand, and engage in skill-based activities EDUCATION & EXPERIENCE REQUIREMENTS Required: * High School Diploma or equivalent * Reliable transportation * Valid driver's license * Ability to pass background checks Preferred: * Bachelor’s degree in Human Services or related field * Experience with children, adolescents, or adults (based on program) * Behavioral health or PRP experience
