Position Overview The Registered Nurse (RN) plays a vital role in delivering safe, efficient, and patient-centered care within our primary care setting. You will collaborate closely with providers, medical assistants, and interdisciplinary care teams to support patients across the lifespan. Key Responsibilities * Provide direct nursing care and clinical support to patients of all ages * Assist with triage, patient education, care coordination and patient follow-up * Administer immunizations and medications in accordance with clinical protocols * Assist providers with examinations, procedures, and treatments within scope * Collect and process laboratory specimens and perform point of care testing * Support chronic disease management and preventative care initiatives * Collaborate with care teams to ensure an exceptional patient experience * Maintain accurate and timely documentation in the electronic health record * Participate in quality improvement, infection control, and patient safety initiatives Qualifications * Current RN license in New Hampshire (or ability to obtain) * Strong communication and collaboration skills * Commitment to compassionate, respectful, and inclusive care * Experience in primary care, outpatient, or community health is preferred but not required Benefits at CCFHS We are proud to offer a comprehensive benefits package for eligible employees.
