As a valued member of our SET team, you'll step in to provide essential support when it’s needed most, ensuring our clients receive reliable and responsive care. Key Responsibilities: On-Call Coverage: * Be available 6:00 AM - 11:00 PM on your selected weekday (from available options). * Participate in an on-call rotation every 5th weekend to cover any last-minute call-offs. * 2 holidays Personal Care & Assistance: * Help clients with bathing, dressing, grooming, and toileting, ensuring dignity and comfort. * Assist with mobility support, including transfers, walking, and repositioning. Health & Wellness Monitoring: * Provide medication reminders and report any health concerns. * Monitor and document changes in the client’s physical and mental condition. Household Support & Companionship: * Perform light housekeeping, including cleaning, laundry, and meal preparation. * Offer emotional support and companionship to enhance clients’ overall well-being. * Ensure a safe home environment by identifying and addressing potential hazards. Accurate Documentation & Communication: * Maintain detailed records of care provided, client condition changes, and any incidents during shifts. What We’re Looking For: Skills & Qualifications: * Strong interpersonal and communication skills. * Ability to adapt to changing client needs and follow individualized care plans. * Basic knowledge of medical procedures and terminology. * Compassionate, patient, and empathetic approach to caregiving. Physical & Other Requirements: * Ability to lift, move, and assist clients as needed. * Valid driver’s license and reliable transportation (if required for travel). * Ability to pass a background check (as applicable). Preferred Experience & Certifications: * 1+ year of experience as a Home Health Aide (HHA) or CNA (Required). * HHA or CNA license (Required). * Driver’s License & Auto Insurance (Required). * CPR Certification (Preferred).
