Part time Registered Nurse Case Manager Anticipated Territory : Sacramento County Schedule: Every Saturday and Sunday Full Days. Would consider Saturday/Sunday plus 3 week days.     What you’ll do: Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including: * Conduct Start of Care (SOC) Admissions - OASIS * Conduct follow ups, evaluation visits as needed in designated territory * Make home visits to patients in designated territory * Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures * Monitor clients’ conditions, reporting changes to Clinical team * Collaborate with physicians and agency staff to deliver the best care possible * Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols. * Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
